"SCHEDULE IV Deaths of Officers and Enlisted Men, which have occurred while in the Military or Naval Service of the United States or from wounds or disease acquired in said service since April, 1861, reported by the families to which the deceased belonged when at home in the __________ of __________ County of __________, N.Y."  Prepared June 3, 1865.
 

        All of these deaths are war related, and varied from illness to wounds received in battle.  Causes of death and regimental data are given in the original schedule but are not copied here.  The names of 86 men appear on this list for the three towns (Myron Gibbs appears twice on the schedule; from Fenner and Cazenovia).  Those soldiers that returned home to die are also referenced in the combined list.
        Clarification of southern place names, which were clearly unknown to the enumerator, has been made (Coal Harbor = Cold Harbor, Newburn = New Berne, etc.) and state abbreviations have been added to all known locations.
        The original lists gives regiments, but I did not copy this information.  I have found this information on Susan Greenhagen's master list of Madison County Civil War Soldiers, which was built upon the foundation laid by Madison County Historian Isabel Bracy and I have added other information as I have found it and which has been sent to me by other researchers.
