The form below will help you format your look-up request, and will e-mail it automatically to the owner of the resource you have just selected.
Please fill out the form completely, particularly those items marked [REQUIRED]. Missing or unclear information may well result in you receiving no answer to your request.
Please make sure your return email address is correct - our lookup volunteers receive countless requests they are unable to fulfill because of invalid email addresses. It is YOUR responsibility to provide the correct email address. Provide the email address within your message, too, just to be sure.
If your email service has a "whitelist" [where you accept emails only from those on an approved list] please add the lookup volunteer's email address to that whitelist - otherwise, they will not be able to respond to your request in a timely manner.

Please limit the number of surnames or resources you want looked up by any one owner. Many owners receive up to 300 e-mail messages per day. Since they are doing look ups as volunteers, it is important not to overuse their service.
Be sure to notice whether the owner is requesting an SASE. If the owner is making copies of information and sending it to you, be sure to ask how much it will cost, so that you can reimburse the owner for his or her expenses.
Please be sure to thank your lookup volunteer, whether or not they were able to locate your information. They are performing a valuable service - it is always nice to know their time and effort are appreciated.

When requesting a look-up, please let me know WHICH surname you are seeking information on and I will look them up and tell you the first names of the individuals associated with those surnames. (Please limit your requests to reasonable lengths!) If any of the names found are your relatives or people you would like more information on, I will transcribe the biographical sketches of the individuals (usually 2-4 pages each) for free and e-mail them to you. Unfortunately, I have had to rescind my previous offer to also make photocopies of the pages and mail them due to the stress this has created on the book itself. This book is nearly 100 years old and cannot withstand continuous photocopying and the repeated bending back of its spine associated with making copies. I have already received numerous requests, and unfortunately I cannot continue to place those requests on the antiquated condition of this resource. I can send a copy of the cover of the book itself, as well as the title page of the book, the publisher's note which substantiates the accuracy of the information contained, and a copy of the index which lists all individuals in the book and snail-mail them to researchers for a small fee to cover photocopying costs and postage; these copies already exist so will therefore cause no additional undue stress to the book.
